CANCELLATION POLICY

Please know that we reserve the right to charge a $50 cancellation fee from patients who fail to show up for an appointment without giving 24 hours notice to cancel or reschedule. If you are canceling with short notice it is highly likely that your appointment time could have been booked with another patient if you had given more than 24 hours notice.

FEES (CASH PRICE)

First office visit: $250

Return office visit: $175

Massage Therapy: $150 for 60 minutes

Laser: $30 for 15 minutes, $60 for 30 minutes

These rates are “time of service cash rates.” Claims billed through insurance will be processed per each insurance company’s Fee Schedule.

You can contact your insurance company to request their fee schedule.

Our clinic most commonly bills the following cpt codes, however, others will be processed as appropriate: 99203-99205, 99213-99215, 97140, 97810, 97811, 98926-98928

    Patients are responsible for learning about their own benefits and coverage prior to visits. Please check with your insurance company so you will be informed about your coverage and will not be surprised by bills you may receive after your visits.

​We may verify certain benefits in our office using the same online tools that are available to you through your insurance provider. However, this is a courtesy that is dependent upon staffing availability and is not guaranteed.
